What is Metastatic Spinal Cord Compression (MSCC)

Metastatic Spinal Cord Compression (MSCC) refers to compression of the spinal cord and/or cauda equina by pathological vertebral collapse and/or direct tumour expansion. This can result in partial or complete loss of neurological function and is therefore classed as an oncological emergency. Audits and studies have shown that the best outcomes for patients with MSCC are achieved following early recognition, rapid referral and appropriate timely management.(1) The MSCC Service is hosted at The Clatterbridge Cancer Centre (CCC) and provides a single point of contact for the regional emergency spinal oncology pathway. The purpose of the MSCC Service is to provide support and guidance to healthcare professionals in order to streamline diagnostic, treatment and rehabilitation processes for patients with MSCC or suspected MSCC. The service supports Cheshire and Merseyside’s primary care services, secondary care acute trusts/urgent care pathways, haematology teams, hospice/community teams, the Walton Spinal centre and CCC. The National Institute for Health and Clinical Excellence (NICE) guidance ‘Spinal metastases and metastatic spinal cord compression” published in 2023 (1), forms the basis of this pathway.

Meet the Team

Roles and Responsibilities

MSCC Lead

CORD Consultant

A Consultant Clinical Oncologist who is assigned responsibility for the oncology management of patients with suspected or confirmed MSCC on a weekly roational basis.

MSCC Coordinator

To coordinate the care of all patients referred to the service with an oncological spinal emergency / urgency, ensuring that key information about the patient is received and discussed in an MDT to provide the right management plan.

MSCC Cancer Support Worker

To support in developing a patient centered personalised care plan with Holistic Needs Assessments (HNA) with both the patient and family members or carers.

MSCC Cancer Support Workers will also support transfers to CCCL and facilitate repatriation back to the referring trust.

MSCC Support Coordinator

Coordinates the administrative, data, audit and governance processes with the MSCC service and supports the MSCC Coordinators with operational tasks.

MSCC Physiotherapy Rehab

To offer guidance and advice on assessment, mobilisation, rehabilitation and brace guidance for patients.


MSCC Clinical Analyst

To develop and introduce new digital solutions to streamline existing processes and improve the efficiency for the MSCC pathway.

Care Plan

Surgery

You may be considered for surgery when you come through the MSCC Pathway. Discussions will take place between the MSCC Team at Clatterbridge and the Spinal Team at the Walton Neurological Centre to ensure that the best care plan is given.

When you arrive at Clatterbridge Cancer Centre, you will be invited to complete a Holistic Needs Assessment (HNA) by our Cancer Support Worker.

You will be able to talk through your needs and concerns and then agree on a plan for your care and support needs.

Systemic Treatment

Systemic Treatments are drug therapies that work throughout the whole of your body. This can be given as an injection, infusion or oral medication. They include chemotherapy, hormonal treatment, targeted therapy or immunotherapy.

Acute Oncology Team

The Acute Oncology Team provides support and expert advice to the team that are caring for you, on a wide range of cancer related issues. The team ensures that you have the right tests, treatment, emotional support, information and advice whilst you are in hospital and to ensure you have appropriate follow up once discharged.

Enhanced Supportive Care (ESC)

Enhanced Supportive Care helps people with cancer to cope with the impact of the illness and the treatment they are receiving.
