Information issue

Helen Porter, Director of Nursing & Quality at The Clatterbridge Cancer Centre NHS Foundation Trust, said: “An automatic data update to some of our patient records inadvertently resulted in a small percentage (1.8%) containing the wrong information. We have a very strong commitment to patient safety and suspended use of this information until we had identified the affected records.

“That has now been completed and we have put extra resources and additional secure systems in place to ensure clinicians have the correct clinical information for their patients. This means any potential risk to patient care is minimised. We’re working closely with other NHS trusts in the local network to ensure a high standard of patient care is maintained. The cause of the problem has now been addressed. It was due to an error in the way we uploaded some data into our master patient index and we are also carrying out a thorough review to ensure it could not happen again.”

Note: 1,598 records were affected. This accounts for 1.8% of our current patient records. We are writing to all the affected patients so there is no undue anxiety. Patients who have any queries or concerns can call 0151 482 7752.
